Purpose of the Study:

  • To investigate the chemical composition of gallstones in Ghanaian patients.
  • To correlate external stone appearance with chemical composition.
  • To analyze the impact of biliary bacteriology on gallstone composition.

Main Methods:

  • Gallstones from 67 patients undergoing cholecystectomy were collected.
  • Stones were classified by appearance (cholesterol, black pigment, brown pigment).
  • Chemical analysis determined cholesterol content, reclassifying stones as cholesterol-poor, intermediate, or cholesterol-rich.

Main Results:

  • External appearance was reliable for cholesterol and black pigment stones but not brown pigment stones.
  • 52% of stones were cholesterol-poor, 13% intermediate, and 34% cholesterol-rich.
  • Cholesterol-rich gallstones were more frequent in women; biliary bacteriology showed no significant impact on stone composition.

Conclusions:

  • The majority of Ghanaian gallstones are cholesterol-poor, favoring surgical intervention over dissolution therapy.
  • Contrary to common belief, cholesterol-rich gallstones are present in West Africa (34% in this study).
  • Further research into the etiology and optimal treatment of gallstones in Ghana is warranted.
