Area of Science:

  • Psychiatry
  • Clinical Psychology
  • Neuroscience

Background:

  • Panic disorder (PD) is considered heterogeneous.
  • Depersonalization during panic attacks may indicate a distinct PD subgroup.

Purpose of the Study:

  • To analyze the differential features of PD patients with depersonalization.
  • To investigate if depersonalization constitutes a unique PD subtype.

Main Methods:

  • 274 PD patients were assessed using the Structured Clinical Interview for DSM-III-R (SCID-UP-R).
  • Patients were divided into groups based on the presence or absence of depersonalization.
  • Clinical scales included Hamilton Anxiety and Depression Rating Scale (HARS/HDRS), Marks and Mathews Fears and Phobia Scale, and Panic-Associated Symptom Scale (PASS).

Related Experiment Videos

Main Results:

  • 66 patients (24.1%) reported depersonalization during panic attacks.
  • The depersonalization group was younger with an earlier age of onset.
  • This group exhibited more severe PD, including more attacks, worse functioning, and higher symptom scores.
  • Increased comorbidity with specific phobia was observed in the depersonalization group.

Conclusions:

  • Depersonalization during panic attacks may define a distinct and more severe subtype of Panic Disorder.
  • Findings support the heterogeneity of PD and the clinical significance of depersonalization.
