Disseminated small nodular cutaneous sarcoidosis, a rare skin condition, effectively treated with clofazimine. This sarcoidosis variant shows a good prognosis and responds well to clofazimine therapy.

Area of Science:

  • Dermatology
  • Immunology
  • Internal Medicine

Background:

  • Sarcoidosis is a multisystem inflammatory disease characterized by non-caseating granulomas.
  • Cutaneous sarcoidosis presents a wide spectrum of clinical manifestations.
  • Disseminated small nodular cutaneous sarcoidosis is a rare subtype requiring specific diagnostic and therapeutic approaches.

Observation:

  • An 83-year-old woman presented with a 6-month history of itchy papules on her back, spreading to proximal limbs despite corticosteroid treatment.
  • Physical examination revealed scattered red-brown maculopapular lesions, some confluent, with a greyish-yellow hue on diascopy.
  • Skin biopsy confirmed granulomatous epithelioid cell inflammation in the corium without central necrosis.

Findings:

  • Laboratory investigations, including erythrocyte sedimentation rate, angiotensin-converting enzyme (ACE), and calcium levels, were within normal limits.

Related Experiment Videos

  • The diagnosis of disseminated small nodular cutaneous sarcoidosis was established after excluding systemic involvement.
  • Treatment with clofazimine resulted in complete and lasting healing of the cutaneous lesions.
  • Implications:

    • Disseminated small nodular cutaneous sarcoidosis has a favorable prognosis.
    • Clofazimine is an effective and well-tolerated therapeutic option for this condition.
    • Early diagnosis and appropriate treatment are crucial for managing cutaneous sarcoidosis.
