Background:

  • Acute ischemic stroke is a leading cause of death and disability.
  • Piracetam is a nootropic drug with potential neuroprotective properties.
  • Evidence regarding piracetam's efficacy in acute ischemic stroke is limited.

Purpose of the Study:

  • To evaluate the effect of piracetam on case fatality and functional outcomes in patients with acute ischemic stroke.
  • To synthesize evidence from randomized controlled trials examining piracetam's use within 48 hours of stroke onset.

Main Methods:

  • Systematic review and meta-analysis of randomized controlled trials (RCTs) following the Cochrane Stroke Group strategy.
  • Inclusion criteria: presumed ischemic stroke, treatment within 48 hours.
  • Endpoints: case fatality and functional outcome.

Main Results:

  • Three RCTs were included, with one trial contributing over 97% of the data (501 patients treated with piracetam, 501 controls).
  • Piracetam showed a nonsignificant 31% increase in the odds of death (95% CI -5% to 81%).
  • No significant difference in functional outcome was reported in the largest study; manufacturer data suggested a nonsignificant trend towards reduced dependency.

Conclusions:

  • Current evidence does not support the routine use of piracetam in acute ischemic stroke.
  • A potential beneficial effect of piracetam cannot be entirely excluded.
  • Further well-controlled clinical trials are warranted to clarify piracetam's role in managing acute ischemic stroke.
