Extra-articular features in early rheumatoid disease.

A Fleming, S Dodman, J M Crown

    British Medical Journal
    |May 22, 1976

    Summary

Early rheumatoid disease can present with common extra-articular features like hand-muscle wasting and rheumatoid nodules. Being underweight at onset indicates a more severe rheumatoid disease outcome.

    Area of Science:

    • Rheumatology
    • Clinical Medicine

    Background:

    • Rheumatoid disease (RD) is a chronic inflammatory disorder.
    • Extra-articular manifestations (EAMs) are common in RD and impact prognosis.
    • Understanding early EAMs is crucial for predicting disease severity.

    Purpose of the Study:

    • To prospectively assess the incidence of EAMs in patients with early rheumatoid disease.
    • To identify common and rare EAMs in the initial stages of RD.
    • To determine if early EAMs correlate with disease severity.

    Main Methods:

    • Prospective study of 102 patients with rheumatoid disease within the first year of onset.
    • Regular follow-up every four months for a mean of 4.5 years.
    • Systematic assessment for the presence of various extra-articular features.

    Main Results:

    • Common early EAMs included hand-muscle wasting, carpal tunnel syndrome, lymphadenopathy, ankle swelling, and rheumatoid nodules.
    • Less common early features included hepatomegaly, being underweight, conjunctivitis, and skin transparency.
    • Rare early EAMs comprised scleromalacia, temporal artery involvement, neuropathy, and organ complications.
    • Being underweight was significantly associated with a more severe outcome.

    Conclusions:

    • Early rheumatoid disease exhibits a distinct pattern of extra-articular features.
    • Certain EAMs are more prevalent in the early disease course.
    • Identifying underweight patients early may predict a poorer prognosis in rheumatoid disease.
