Summary

Nailfold video capillaroscopy revealed reduced capillary density in psoriasis patients with nail or joint involvement. Capillary dimensions were smaller in psoriatic arthritis patients, suggesting vascular injury contributes to disease pathogenesis.

Background:

  • Microvascular changes are implicated in the pathogenesis of psoriasis and psoriatic arthritis.
  • Nailfold video capillaroscopy, an extension of nailfold microscopy, studies in vivo microcirculation.
  • Previous studies on nailfold capillaries in psoriasis patients yielded conflicting results.

Purpose of the Study:

  • To investigate nailfold capillary abnormalities in psoriasis patients, particularly those with adjacent nailfold pathology.
  • To test if quantitative or qualitative capillary changes are more evident in subjects with distal interphalangeal (DIP) joint changes and/or nail dystrophy.

Main Methods:

  • Recruited 44 patients with psoriasis (varying degrees of nail and DIP joint involvement) and 44 age- and sex-matched controls.

    • No significant difference in capillary dimensions between psoriasis patients (with or without nail changes) and normal controls.
    • No specific morphological abnormalities were noted in any patient group.

    Conclusions:

    • Diminished nailfold capillary bed density and dimensions in psoriasis patients suggest vascular injury may contribute to psoriatic arthritis pathogenesis.
    • Unlike some prior studies, no specific morphological pattern of nailfold capillaries was identified in psoriasis patients compared to controls.
