Area of Science:

  • Endocrinology
  • Genetics
  • Reproductive Medicine

Background:

  • Persistent Müllerian Duct Syndrome (PMDS) is a rare intersex condition characterized by the presence of Müllerian duct remnants in XY males.
  • This syndrome, first described in 1939, has approximately 100 reported cases worldwide.

Purpose of the Study:

  • To report a case of PMDS presenting with inguinal hernia.
  • To discuss diagnostic and management strategies for PMDS.

Main Methods:

  • A 22-year-old male presented with right inguinal pain and scrotal heaviness.
  • Surgical exploration revealed bilateral testes, a bicornuate uterus, and rudimentary fallopian tubes, which were removed.
  • Hernia repair was also performed.

Main Results:

Related Experiment Videos

  • The patient was diagnosed with Persistent Müllerian Duct Syndrome.
  • Surgical intervention included removal of Müllerian structures and hernia repair.

Conclusions:

  • Management of PMDS is challenging due to its rarity.
  • Preoperative diagnosis using karyotyping, laparoscopy, and ultrasonography is recommended for suspected cases.
  • Preserving secondary male sex characteristics and considering androgen replacement therapy are vital post-gonadectomy.
