Area of Science:

  • Surgical Care and Quality Improvement
  • Health Services Research

Background:

  • Ambulatory surgery has become a standard model for surgical care in Spain and other Western countries.
  • Objective quality indicators are essential for enhancing surgical outcomes, minimizing patient morbidity, optimizing resource use, and ensuring patient and family satisfaction.

Purpose of the Study:

  • To establish a list of quality assessment indicators for ambulatory surgery.
  • To discuss the need for consensus on patient and procedure complexity in ambulatory surgery.

Main Methods:

  • A prospective, descriptive study involving 833 patients undergoing ambulatory surgery across six specialties (General Surgery, Ophthalmology, Orthopaedics, Urology, Gynaecology, E.N.T.).
  • Data collected included unplanned admission, re-admission, emergency consultations, cancellations, substitutions, and postoperative pain.

Related Experiment Videos

  • Diagnoses and procedures were classified using CIE-9 and DGR systems.
  • Main Results:

    • Ophthalmology was the most frequent specialty (33.2%).
    • Key quality indicators: Unplanned Admission Index 7.2%, Re-Admission Index 0.4%, Emergency Department Consultation 1.6%, Cancellation Index 3.3%, Substitution Index 70%.
    • Primary reasons for unplanned admissions were specific complications (60.1%) and inadequate patient selection (36.6%).

    Conclusions:

    • Systematic application and evaluation of quality indicators are necessary for ambulatory surgery.
    • These indicators can aid in defining national standards to drive continuous improvement in ambulatory surgical care.
