Unusual immunogammopathy maculopathy.

Summary

Unusual macular detachment can indicate underlying serum immunogammopathies like multiple myeloma. Early diagnosis through serum protein electrophoresis is crucial for patients with atypical macular conditions.

Background:

Serum immunogammopathies are conditions involving abnormal proteins in the blood.
These conditions can manifest with diverse systemic and ocular symptoms.

Observation:

Atypical macular detachment was observed in three patients.
Associated findings included subretinal precipitates and signs of serum hyperviscosity.

Findings:

Fluorescein angiography revealed macular hypofluorescence.
No leakage from retinal vasculature or pigment epithelium was detected within the macular elevation.

Implications:

Atypical macular detachment may signal underlying immunogammopathies, including multiple myeloma and Waldenström's macroglobulinemia.
Consider serum protein electrophoresis and hematology consultation for affected patients.
