[The diagnosis of autonomic disorders in premature newborns]
Insights
A new diagnostic algorithm identifies sympathicotonia and parasympathicotonia in newborns. This method uses 11 indices for early autonomic disorder detection in infants, avoiding complex tests.
Area of Science:
- Neonatal Medicine
- Autonomic Nervous System Physiology
- Pediatric Diagnostics
Context:
- Autonomic nervous system (ANS) imbalances, specifically sympathicotonia and parasympathicotonia, are critical in neonatal health.
- Accurate and early identification of ANS dysfunction in newborns is essential for timely intervention and improved outcomes.
- Current diagnostic methods can be invasive, costly, and time-consuming, posing challenges in routine neonatal care.
Purpose:
- To develop and validate a novel diagnostic algorithm for identifying sympathicotonia and parasympathicotonia in newborn infants.
- To establish a reliable, non-invasive, and cost-effective method for assessing autonomic balance in neonates.
- To provide a practical tool for clinicians to determine the direction of autonomic disorders in newborns.
Summary:
- A diagnostic algorithm was developed based on observations of 286 premature infants over their first 14 days of life.
- The algorithm incorporates 11 informative indices, with a score of +13 indicating sympathicotonia and -13 indicating parasympathicotonia at a 95% probability threshold.
- This method allows for the determination of autonomic disorder direction in newborns without the need for complex investigations like spectral analysis or hormone measurements.
Impact:
- Enables early and accurate diagnosis of autonomic disorders in newborns, facilitating prompt clinical management.
- Reduces the need for expensive and elaborate diagnostic procedures, making ANS assessment more accessible in neonatal units.
- Provides a valuable tool for understanding and managing sympathicotonia and parasympathicotonia, contributing to improved infant health and development.
Abstract:
A diagnostic algorithm has been worked out for identification of sympathicotonia and parasympathicotonia in newborn infants. The algorithm is calculated on the basis of results of observations carried out in 286 premature infants who had been examined three times during the first fourteen days of their lives. Included in the algorithm are 11 indices of informative value. Score total +13 suggest to us sympathicotonia while -13 indicates parasympathicotonia, with probability threshold being 95%. The above method permits the determination of direction of autonomic disorders in newborn infants, in which case there is no necessity whatever to conduct elaborate and expensive investigations (spectral analysis of the heart's rhythm, measurement of hormones and mediators.
