Summary

This study describes a rare case of selective paraspinal muscle amyotrophy in a 56-year-old female. Findings suggest it may represent a new form of benign focal amyotrophy.

Background:

  • Paraspinal muscle weakness can present with various etiologies.
  • Differentiating causes of localized muscle atrophy is clinically important.

Observation:

  • A 56-year-old female presented with mild low back pain and selective thoracic and lumbar paraspinal muscle atrophy.
  • Electromyography (EMG) showed paraspinal muscle fibrillations with normal limb EMG.
  • Magnetic resonance imaging (MRI) confirmed paraspinal muscle atrophy; gluteus maximus biopsy was normal.

Findings:

  • Paraspinal muscle biopsy revealed neurogenic features, consistent with selective amyotrophy.
  • The patient's condition, selective thoracic and lumbar paraspinal amyotrophy, is discussed as a potential addition to benign focal amyotrophy.

Implications:

  • This case expands the spectrum of benign focal amyotrophy.
  • Highlights the importance of considering selective paraspinal amyotrophy in the differential diagnosis of low back pain and muscle weakness.
