Patients are moving more rapidly from one healthcare setting to another. Often these patients have more complicated therapies and access devices and more potential for adverse occurrences. To provide care for these patients across healthcare settings, institutions need to plan a framework for delivery of quality care and services while minimizing the potential for complications.
