Effect of age at disease onset in the clinical profile of spondyloarthritis: a study of 1424 Brazilian patients.

Clinical and experimental rheumatology·2012

Area of Science:

  • Rheumatology and Gynaecology
  • Reproductive Health in Autoimmune Diseases

Background:

  • Systemic sclerosis (SSc) is a complex autoimmune disease affecting connective tissues.
  • Understanding the gynaecologic history and reproductive outcomes in SSc patients is crucial for patient care.

Purpose of the Study:

  • To analyze the gynaecologic history of Brazilian patients with SSc.
  • To compare pregnancy outcomes before and after SSc onset and across SSc variants.
  • To assess the impact of pregnancy on SSc disease progression.

Main Methods:

  • Retrospective analysis of 150 female SSc patients over 18 years old.
  • Data collection on pregnancies, deliveries, fetal deaths, dyspareunia, and menopause.
  • Subdivision of data into pregnancies before and after SSc onset.

Related Experiment Videos

Main Results:

  • Fertility rate was higher in limited SSc (3.6) than diffuse SSc (3.1) patients.
  • No increased risk of infertility or spontaneous abortions observed in SSc patients.
  • Pregnancies after SSc onset remained stable in 72% of cases, with 14% worsening and 14% improving.
  • Fetal death rate was 14% (58/406 pregnancies), with most occurring before SSc onset.

Conclusions:

  • SSc patients exhibit normal fertility and pregnancy outcomes, without increased risks.
  • Pregnancy generally has a stable or non-worsening effect on SSc progression.
  • The study provides valuable insights into reproductive health for women with SSc.
