    Summary

    Solvent/detergent virus-inactivated plasma (S/D-VIP) demonstrated similar effects to fresh frozen plasma (FFP) in patients, with no significant differences in coagulation markers after transfusion. This suggests S/D-VIP is a viable alternative to FFP.

    Background:

    • Reduced levels of protein S (PS) and alpha(2)-antiplasmin (alpha(2)-AP) in solvent/detergent virus-inactivated plasma (S/D-VIP) may alter coagulation balance.
    • Patients with coagulopathy, liver disease, DIC, polytrauma, or on extracorporeal circulation were studied.

    Purpose of the Study:

    • To compare the hemostatic effects of S/D-VIP and fresh frozen plasma (FFP) in transfused patients.
    • To investigate the impact of S/D-VIP on coagulation factors and inhibitors.

    Main Methods:

    • 40 patients received either FFP (n=23) or S/D-VIP (n=17).
    • Markers of activated coagulation (MAC) and key inhibitors (antithrombin, protein C, PS, alpha(2)-AP) were measured pre- and 1 hour post-transfusion.
    • Statistical analysis included Wilcoxon's paired test and Mann-Whitney test.

    • All measured inhibitors increased post-transfusion in both groups, except for PS.
    • S/D-VIP group showed a significantly higher increase in antithrombin, protein C, and alpha(2)-AP compared to the FFP group.
    • No significant differences were observed between the FFP and S/D-VIP groups for pre/post values or MAC levels.

    Conclusions:

    • S/D-VIP plasma functions comparably to FFP in the studied patient population.
    • The study indicates S/D-VIP is a safe and effective alternative to FFP under the employed conditions.
    • No significant imbalance in coagulation factors or inhibitors was detected with S/D-VIP transfusion.
