Area of Science:

  • Orthopedic Surgery
  • Traumatology
  • Biomedical Engineering

Background:

  • Humeral shaft fractures are common injuries requiring effective stabilization.
  • Unreamed antegrade interlocking nailing is a surgical technique for humeral fractures.

Purpose of the Study:

  • To evaluate the outcomes of unreamed antegrade interlocking nailing for humeral fractures.
  • To assess fracture consolidation, functional recovery, and complications.

Main Methods:

  • Prospective clinical study of 109 patients with 111 humeral fractures.
  • Fractures stabilized using unreamed antegrade interlocking nailing.
  • Follow-up evaluation at a mean of 24.2 months postoperatively.

Main Results:

  • Mean fracture consolidation time was 12.3 weeks; 5 non-unions occurred.
  • 19 patients had shoulder motion limitation (mean 20%); 8 had permanent shoulder pain.
  • Rotator cuff lesions were identified in 6 patients, linked to shoulder pain. Prominent nail tips caused 5 rotator cuff lesions.
  • Radial nerve palsies resolved in 8/9 patients within 4.3 months.

Conclusions:

  • Unreamed antegrade interlocking nailing is safe for humeral fractures, achieving good consolidation and early mobilization.
  • Preventing rotator cuff lesions through careful suturing and nail tip countersinking is vital for avoiding shoulder pain and dysfunction.
