Background:

  • Hysterosalpingo contrast sonography (HyCoSy) is often favored over hysterosalpingography (HSG) for assessing Fallopian tube patency.
  • HyCoSy is non-ionizing, visualizes uterus and ovaries, and is generally considered safe, quick, and easy.

Purpose of the Study:

  • To compare the diagnostic accuracy, costs, and patient discomfort of HyCoSy and HSG.
  • To evaluate the safety and tolerability of HyCoSy in a real-world clinical setting.

Main Methods:

  • 118 women underwent HyCoSy, and 116 underwent HSG by the same operators.
  • Patient discomfort was graded on a 0-4 scale.
  • 15 patients had both procedures for direct comparison.
  • Costs were estimated.

Main Results:

  • 23 out of 118 HyCoSy patients experienced severe pain or vasovagal reactions, with seven requiring resuscitation.
  • HSG had no severe adverse reactions in 116 patients.
  • HyCoSy had a higher rate of severe discomfort compared to previous reports and HSG.
  • Diagnostic accuracy, cost, and discomfort of HyCoSy were found to be unfavorable compared to HSG.

Conclusions:

  • HyCoSy can cause significant pain and adverse reactions, contrasting with previous literature.
  • HSG appears to be a safer and more favorable option for evaluating Fallopian tube patency.
  • Further investigation into HyCoSy's adverse reaction mechanisms is warranted.
