    Summary

    This study examines cryptogenic fibrosing alveolitis, finding pleural involvement and restrictive defects in most patients. One patient showed significant improvement, suggesting potential for recovery in cryptogenic fibrosing alveolitis.

    Background:

    • Cryptogenic fibrosing alveolitis (CFA) is a progressive lung disease.
    • Understanding the temporal evolution of CFA is crucial for effective treatment.

    Purpose of the Study:

    • To describe clinical, radiographic, histologic, and pulmonary function features in CFA patients.
    • To propose a temporal evolution scheme for CFA based on histologic patterns.

    Main Methods:

    • Analysis of clinical, radiographic, histologic, and pulmonary function data from 11 CFA patients.
    • Immunofluorescent studies on lung biopsies.
    • Correlation of histologic patterns with symptom onset to biopsy interval.

    Main Results:

    • Exertional dyspnea was common; 8/11 patients had pleural involvement and restrictive defects.
    • One patient demonstrated marked improvement across all parameters.
    • Immunofluorescence studies showed no evidence of immunologic damage in lung biopsies.

    Conclusions:

    • A tentative scheme for CFA temporal evolution is proposed, involving direct progression to diffuse interstitial pulmonary fibrosis (DIPF) or initial alveolar/mural patterns progressing to DIPF.
    • Lung injury may initiate varied reaction patterns that can ultimately lead to DIPF.
