Summary

Basaloid neoplasms in nevus sebaceus (NS) are predominantly benign trichoblastomas (TB), not basal cell carcinomas (BCC). This study clarifies diagnostic criteria for these skin lesions.

Background:

  • Nevus sebaceus (NS) frequently harbors basaloid neoplasms, with their classification debated between basal cell carcinoma (BCC) and trichoblastoma (TB).
  • Distinguishing between BCC and TB in NS is crucial for accurate diagnosis and patient management.
  • Previous studies have varied in their classification of these lesions.

Purpose of the Study:

  • To histopathologically characterize basaloid neoplasms arising in NS.
  • To differentiate between trichoblastomas (TB) and basal cell carcinomas (BCC) in NS using current criteria.
  • To provide a large series for better understanding of these entities.

Main Methods:

  • Review of 316 NS cases over 19 years for basaloid neoplasms.
  • Categorization of 24 identified basaloid neoplasms into TB or BCC based on histopathologic criteria.
  • Comparison with 37 solitary TB cases.

  • 91.6% of basaloid neoplasms in NS were classified as TB (22 cases), and 8.4% as BCC (2 cases).

  • No recurrences or metastases were observed in follow-up for TB and BCC cases.

Conclusions:

  • The majority of basaloid neoplasms in NS are histopathologically consistent with TB.
  • A minority of cases may present diagnostic challenges due to overlapping features or co-occurrence with other neoplasms.
  • Clear morphologic criteria aid in distinguishing TB from BCC in NS.
