A rare extralacrimal dacryolith, a stone in the tear drainage system, was found externally on a patient's nose. This case highlights unusual presentations of dacryolithiasis.

Background:

  • Dacryoliths are calcified concretions typically found within the lacrimal drainage system.
  • Extralacrimal dacryoliths, located outside the lacrimal sac and ducts, are exceptionally rare.

Observation:

  • A 43-year-old woman presented with a firm, slowly enlarging nasal mass, initially asymptomatic.
  • The patient denied any history of lacrimal outflow obstruction or dacryocystitis.

Findings:

  • Surgical excision revealed a dacryolith situated external to the lacrimal sac and duct, adhered to its lateral wall.
  • Histopathology confirmed a dacryolith surrounded by chronic inflammation, lacking epithelial lining.

Implications:

  • This case suggests that dacryoliths can form within the lacrimal sac and subsequently migrate into adjacent soft tissues.
