    Long-term hemodialysis for kidney failure may increase cancer risk. Uremia-induced immunosuppression might contribute to reticulosarcoma development and de-novo malignancies in transplant patients.

    Background:

    • Polycystic kidney disease (PKD) is a common cause of end-stage renal disease requiring renal replacement therapy.
    • Uremia, a state of kidney dysfunction, is associated with altered immune function.

    Observation:

    • A case of reticulosarcoma with cerebral involvement is presented in a patient undergoing maintenance hemodialysis for PKD.
    • The patient developed cancer during the course of long-term hemodialysis treatment.

    Findings:

    • Reticulosarcoma, a type of non-Hodgkin lymphoma, occurred in the context of chronic hemodialysis.
    • Cerebral involvement suggests advanced disease progression.

    Implications:

    • Uremia-induced immunosuppression may play a role in cancer development during hemodialysis.
    • This case highlights a potential link between immunosuppression in chronic kidney disease patients and increased malignancy risk.
    • Findings suggest a need for vigilance regarding de-novo malignancies in patients with end-stage renal disease and those who have received kidney transplants.
