We describe a rare intrasellar epidermoid cyst case in a 61-year-old man with consciousness disturbance. Surgical removal revealed stratified squamous epithelium, confirming the diagnosis of this unusual pituitary region tumor.

Area of Science:

  • Neurosurgery
  • Endocrinology
  • Pathology

Background:

  • Intrasellar epidermoid cysts are exceptionally rare pituitary region tumors.
  • Clinical presentation can be nonspecific, including altered consciousness and fatigue.

Observation:

  • A 61-year-old male presented with disturbed consciousness and fatigability.
  • Laboratory tests revealed panhypopituitarism.
  • Magnetic Resonance Imaging (MRI) identified a cystic lesion in the intrasellar region.

Findings:

  • The tumor was successfully resected via a transsphenoidal approach.
  • Histological examination confirmed an epidermoid cyst.
  • The cyst wall exhibited characteristic stratified squamous epithelium with keratohyaline granules.

Implications:

  • This case highlights the importance of considering rare cystic lesions in the differential diagnosis of intrasellar masses.
  • Transsphenoidal surgery is a viable option for managing symptomatic intrasellar epidermoid cysts.
  • Accurate pathological diagnosis is crucial for understanding the nature of these tumors.
