Summary

Splenosis, splenic implants following splenectomy, can cause gastrointestinal bleeding. Early diagnosis and surgical removal of symptomatic implants are crucial for patient recovery.

Background:

  • Splenosis, characterized by splenic tissue implants, often follows splenectomy due to trauma.
  • It can mimic other conditions, complicating diagnosis.

Observation:

  • A 48-year-old male with recurrent gastrointestinal bleeding and anemia presented with non-diagnostic endoscopic evaluations.
  • Angiography revealed apparent arteriovenous malformations, leading to exploratory laparotomy.

Findings:

  • Exploratory laparotomy identified numerous splenic implants in the small and large bowels, with some eroding the mucosa and causing bleeding.
  • Surgical excision of these implants successfully halted the bleeding. A renal cell carcinoma was also resected.

Implications:

  • Splenosis, though often asymptomatic, must be considered in the differential diagnosis for unexplained bleeding or masses in post-splenectomy patients.
