A patient developed cerebrovascular insult after varicose vein surgery due to a thrombus in the patent foramen ovale. Treatment with heparin resolved the thrombus, leading to gradual recovery with residual neurological deficit.

Area of Science:

  • Vascular Surgery
  • Neurology
  • Cardiology

Background:

  • Varicose vein surgery can rarely lead to paradoxical embolism.
  • Patent foramen ovale (PFO) is a potential pathway for clot migration.

Observation:

  • A 49-year-old female presented with cerebrovascular insult post-varicose vein surgery.
  • Transesophageal echocardiography revealed a serpentine thrombus trapped in a PFO.

Findings:

  • Intracardiac thrombus in PFO was diagnosed as the cause of cerebrovascular insult.
  • Intravenous heparin treatment led to thrombus resolution and patient recovery.

Implications:

  • Highlights the importance of considering paradoxical embolism in post-operative cerebrovascular events.

Related Experiment Videos

  • Emphasizes the diagnostic role of echocardiography in identifying intracardiac thrombi.
  • Suggests heparin as an effective treatment for such embolic events.
