Background:

  • Macrolide antibiotics like erythromycin are known to potentially prolong the QT interval.
  • QT prolongation can lead to serious cardiac arrhythmias, including torsades de pointes (TdP).

Observation:

  • Two patients developed QT prolongation and TdP while receiving clarithromycin (400 mg/day) for respiratory conditions.
  • Neither patient was taking other medications known to cause QT prolongation.

Findings:

  • This is the first reported association of clarithromycin with TdP in the absence of other confounding drugs.
  • Clarithromycin may independently increase the risk of TdP.

Implications:

  • Clinicians should be aware of the potential cardiac risks associated with clarithromycin.
