    Beclomethasone dipropionate aerosol effectively reduces or replaces systemic corticosteroids for most asthma patients. This therapy improved symptoms and lung function, with many patients reducing or stopping oral prednisone.

    Background:

    • Systemic corticosteroids are common for persistent asthma but have side effects.
    • Alternative therapies are needed to reduce reliance on oral steroids.

    Purpose of the Study:

    • To evaluate beclomethasone dipropionate aerosol as a replacement for systemic corticosteroids in asthma patients.
    • To assess the impact on symptoms, pulmonary function, and steroid dosage.

    Main Methods:

    • A clinical trial involving 41 patients with perennial asthma.
    • Patients were assessed for symptomatic improvement and pulmonary function.
    • Dosage and duration of prior systemic corticosteroid use were recorded.

    Main Results:

    • 10 patients not on long-term steroids showed symptomatic and functional improvement.
    • Of 31 patients on long-term steroids, 12 discontinued prednisone, 15 reduced their dose, and 4 maintained their dose.
    • Successful reduction was more likely in patients with <15 mg daily prednisone for <3 years.
    • Side effects included dry throat (7 patients) and Candida infections (2 patients).
    • 11 patients experienced rhinitis recurrence after steroid reduction.

    Conclusions:

    • Beclomethasone dipropionate aerosol is effective in reducing or replacing systemic corticosteroid therapy for many asthma patients.
    • The therapy maintains or improves lung function while allowing for decreased oral steroid use.
    • Careful patient selection, considering prior steroid dose and duration, may optimize outcomes.
