Summary

Cerebrospinal fluid (CSF) leaks can cause large pituitary glands visible on MRI scans. Surgical repair of these leaks resolves symptoms and normalizes pituitary size, indicating a link between CSF leaks and pituitary changes.

Background:

  • Cerebrospinal fluid (CSF) leaks are a recognized cause of various neurological symptoms.
  • Pituitary enlargement on Magnetic Resonance Imaging (MRI) is an uncommon finding in patients with CSF leaks.

Observation:

  • This study reports on five patients presenting with clinical and MRI evidence suggestive of CSF leak.
  • All patients exhibited large pituitary glands with enhancement on MRI.

Findings:

  • Surgical repair of the CSF leak source was successful in three out of five patients.
  • Following successful surgical intervention, patients experienced symptom resolution and normalization of pituitary size on MRI.
  • Pituitary enlargement in CSF leak cases may represent secondary pituitary hyperemia.

Implications:

  • Pituitary enlargement should be considered an MRI abnormality associated with CSF leaks.
  • Effective surgical management of CSF leaks can reverse associated pituitary changes.
  • Understanding this association aids in diagnosing and managing patients with unexplained pituitary enlargement and potential CSF leaks.
