Obstructing giant colonic diverticulum.

    Summary

    A rare case of a true giant colonic diverticulum causing obstruction was successfully treated with surgery. This finding highlights a flap-valve mechanism as a potential cause for such colonic diverticula.

    • A 75-year-old patient presented with a 6-month history of obstructive symptoms.
    • Endoscopy revealed an obstructing mass 20 cm from the anus, identified as a true giant colonic diverticulum.

    Findings:

    • Microscopic examination confirmed the diverticulum wall possessed all normal colonic layers.
    • Surgical resection of the sigmoid colon with primary anastomosis resolved the patient's obstructive symptoms.

    Implications:

    • This case underscores the importance of considering true giant colonic diverticula in the differential diagnosis of colonic obstruction.
    • A flap-valve mechanism was identified as the cause of this specific true giant colonic diverticulum.
    • Surgical intervention is an effective treatment for symptomatic obstructing true giant colonic diverticula.
