  • Primary melanoma of the urinary bladder is an exceptionally rare malignancy.

Observation:

  • A case of a young female patient with a resected malignant melanoma of the bladder is presented.
  • Extensive investigations were conducted to rule out metastatic disease from other primary sites.

Findings:

  • The bladder melanoma was presumed primary due to the absence of a detectable extravesical primary tumor.
  • Diagnostic challenges in confirming the primary origin of bladder melanoma, even when some criteria are met, are discussed.

Implications:

  • This case may represent the longest survival for primary urinary bladder melanoma.
