Background:

Acquired isolated velopharyngeal palsy is a rare condition, particularly in adolescents.
This report describes the first documented case in Japan.

Observation:

An 18-year-old male presented with conjunctival hyperemia, followed by nasal speech and dysphagia.
Neurological examination revealed isolated right-sided palato-pharyngeal paralysis.
Cerebrospinal fluid analysis showed elevated IgG% and IgG albumin index, suggesting an immune-mediated process.

Findings:

The patient's symptoms improved within weeks following treatment with glycerol and steroids.
No viral etiology was identified through serological testing.
MRI of the head and neck revealed no abnormalities.

Implications:

The findings suggest an immunological basis for acquired velopharyngeal palsy in some cases.
Understanding this rare condition is crucial for accurate diagnosis and management.
Further research into the immunological mechanisms of velopharyngeal palsy is warranted.
