Summary

Tuberculosis of the breast is rare but can mimic other breast conditions. This case highlights the importance of considering tuberculosis in differential diagnoses, even with negative bacterial findings, for effective treatment.

Background:

Breast tuberculosis is an uncommon manifestation of Mycobacterium tuberculosis infection.
It often presents as a palpable mass, mimicking breast cancer or abscess.
Diagnosis can be challenging due to nonspecific clinical and radiological findings.

Observation:

A 47-year-old woman presented with a painful left breast tumor, progressing to abscess and fistula formation.
Mammography revealed a 3.5 cm tumor; abscess culture yielded Staphylococcus aureus.
Histopathology suggested caseous tuberculosis of the mamma, though bacilli were not identified.

Findings:

Despite initial Staphylococcus aureus infection, the underlying pathology was determined to be tuberculosis.
Antituberculous drug therapy was initiated for the confirmed breast tuberculosis.
A skin fistula developed during treatment but resolved with prolonged therapy.

Implications:

This case underscores the need for a high index of suspicion for breast tuberculosis in endemic areas.
Histopathological examination is crucial for diagnosis, even in the absence of positive cultures for Mycobacterium tuberculosis.
Successful management relies on appropriate antituberculous chemotherapy and managing complications like fistulas.
