Summary

Serum alpha-fetoprotein (AFP) is a marker for hepatocellular carcinoma (HCC), but its accuracy is limited in chronic liver disease. Analyzing AFP lectin reactivity offers improved detection of HCC variants in these patients.

Area of Science:

  • Biochemistry
  • Oncology
  • Hepatology

Context:

  • Serum alpha-fetoprotein (AFP) levels normally decrease after birth but are significantly elevated in over 70% of hepatocellular carcinoma (HCC) patients.
  • While benign liver conditions can cause minor AFP elevations, HCC is strongly associated with high AFP concentrations.

Purpose:

  • To evaluate the utility of serum AFP quantification as a diagnostic marker for HCC.
  • To assess the effectiveness of AFP screening in high-risk populations (cirrhosis, hepatitis B carriers).
  • To explore alternative methods for improving HCC detection, especially in cases with moderate AFP elevations.

Summary:

  • Serum AFP is a widely used marker for HCC diagnosis and screening, particularly in individuals with cirrhosis or hepatitis B virus (HBV) infection.

Related Experiment Videos

  • However, the specificity of AFP testing is low in patients with modestly elevated levels (below 400 ng/ml) and in those who are hepatitis B surface antigen-positive (HBsAg+), leading to frequent false positives.
  • Lectin reactivity analysis of AFP shows promise in detecting HCC-specific variants in serum with moderate AFP increases, outperforming simple quantification in chronic hepatitis B patients.
  • Impact:

    • AFP measurement remains crucial for managing benign and malignant liver disorders.
    • Improved diagnostic accuracy for HCC, especially in HBsAg+ patients, is needed.
    • Lectin-based AFP analysis may enhance early HCC detection and patient management in specific populations.
