The open resection technique for acute ascending aortic dissection offers superior early and late survival compared to the graft inclusion technique. Open resection is the preferred method, reducing mortality and pseudoaneurysm formation.

Area of Science:

  • Cardiovascular Surgery
  • Thoracic Surgery
  • Aortic Dissection Research

Background:

  • Acute ascending aortic dissection presents significant surgical challenges with considerable early mortality.
  • Optimal surgical strategies are continuously investigated to improve patient outcomes.

Purpose of the Study:

  • To compare the graft inclusion technique with the open resection technique for acute ascending aortic dissection.
  • To determine the optimal surgical strategy for improved patient survival and reduced complications.

Main Methods:

  • A consecutive series of 193 patients undergoing emergency surgery for acute ascending aortic dissection between 1985 and 1995.
  • Comparison of graft inclusion (n=100) versus open resection (n=93) techniques, including graft replacement details and preoperative risk factors.

Related Experiment Videos

  • Analysis of early mortality, postoperative complications, long-term survival, and reoperation rates.
  • Main Results:

    • Overall early mortality was 24%, with 31% in the graft inclusion group versus 16% in the open resection group (P=0.0154).
    • Survival at 8 years was significantly better with the open technique (P=0.0300).
    • Pseudoaneurysm formation occurred only after graft inclusion (3%), and graft inclusion independently predicted higher early and late mortality.

    Conclusions:

    • The open resection technique is the preferred surgical method for acute ascending aortic dissection, demonstrating superior early and late results.
    • The graft inclusion technique may be considered in specific high-bleeding risk scenarios, with attention to perigraft-space decompression.
    • While late outcomes are generally satisfactory, early mortality remains a concern, highlighting the importance of surgical technique selection.
