    Summary

    Diagnosing childhood tuberculosis (TB) can be challenging, as evidenced by three fatal cases in young children with cavitary pulmonary disease. Negative tuberculin skin tests in these cases highlight the need for considering TB even without typical indicators.

    Background:

    • Tuberculosis (TB) diagnosis in young children presents unique challenges.
    • Cavitary pulmonary disease requires careful differential diagnosis.

    Observation:

    • Three pediatric cases of advanced cavitary pulmonary tuberculosis occurred between 1974-1978.
    • Two children also presented with tuberculous meningitis.
    • Autopsy confirmed all diagnoses, yet tuberculin skin tests were negative in all cases.

    Findings:

    • Negative tuberculin skin tests can occur in children with advanced pulmonary tuberculosis.
    • Establishing an early diagnosis of TB in young children is difficult.

    Implications:

    • Tuberculosis should be a key consideration in the differential diagnosis of pediatric cavitary chest disease.
    • Clinical suspicion for TB is crucial in young children, irrespective of skin test results.
