    Background:

    • Surgical procedures, including nephrectomy, can impact immune system function.
    • Assessing immune response pre- and post-surgery is crucial for understanding patient recovery and potential complications.

    Purpose of the Study:

    • To evaluate the serial changes in both in vivo and in vitro immune function parameters following nephrectomy in healthy renal transplant donors.

    Main Methods:

    • Serial measurements of in vitro immune parameters (lymphocyte counts, B-cell, T-cell counts, mitogen blastogenic response, mixed leukocyte reactivity) were performed.
    • In vivo delayed hypersensitivity skin responses to recall antigens were assessed.
    • Measurements were taken before, during, and after nephrectomy in 12 donors.

    Main Results:

    • In vitro immune functions decreased during and after surgery, reaching a nadir on the evening after nephrectomy, with most returning to near-normal levels by postoperative day five.
    • In vivo delayed hypersensitivity responses declined more gradually, with delayed recovery observed for up to three weeks.
    • Serum immunoglobulin levels remained unchanged throughout the study period.

    Conclusions:

    • Surgery and anesthesia induce a measurable, albeit temporary, depression of immune response.
    • There is an incomplete correlation between commonly used in vitro immune assays and in vivo tests.
    • Despite immune depression, no clinically significant complications were observed in these renal transplant donors.
