Background:

  • Heart rate variability (HRV) is a well-established indicator of autonomic nervous system function, influenced by age.
  • QT variability (QTV), reflecting cardiac repolarization lability, has emerged as a significant predictor of mortality.
  • QTV is hypothesized to correlate with HRV due to shared regulatory mechanisms.

Purpose of the Study:

  • To investigate the relationship between chronological age and QT variability.
  • To explore how age affects the coherence between QT interval and heart rate time series.
  • To assess the influence of posture and breathing on age-related QTV changes.

Main Methods:

  • A novel algorithm was employed to calculate beat-to-beat QT intervals in 34 adults (22-55 years) and 15 children (6-14 years).
  • QT variability measures, including corrected QT variability (QTcvi) and QT variability index (QTvi), were analyzed.
  • Coherence analysis in specific frequency bands (0-0.5 Hz and 0.15-0.5 Hz) and QT-HR time series coherence were assessed under different postures and breathing conditions.

Main Results:

  • QT variability corrected for mean QT interval showed a significant inverse correlation with age.
  • Coherence in the 0-0.5 Hz and high-frequency (0.15-0.5 Hz) bands was also significantly inversely correlated with age.
  • QTvi did not show a significant age relationship in supine posture but exhibited a modest negative correlation in standing posture.

Conclusions:

  • Cardiac repolarization variability, specifically coherence with heart rate, decreases with age.
  • Cardiac vagal activity appears to modulate the coherence between QT and heart rate time series.
  • These findings highlight age-related changes in autonomic control of cardiac repolarization.
