Summary

A rare case of hypodermiasis (botfly infestation) occurred in a child in a cattle-raising area. Early diagnosis is crucial, even with negative initial tests, for effective treatment of this parasitic disease.

Area of Science:

  • Parasitology
  • Dermatology
  • Ophthalmology

Background:

  • Hypodermiasis, a parasitic infestation, is theoretically nearly eradicated.
  • A resurgence is noted in Lower Normandy, a region with significant bovine husbandry.

Observation:

  • A seven-year-old girl presented with eyelid edema, ptosis, and subcutaneous nodules.
  • Initial symptoms showed no eosinophilia or positive serology for hypodermiasis or toxocariasis.
  • Symptoms evolved over weeks, with increasing pain and swelling, later accompanied by eosinophilia and positive hypodermiasis serology.

Findings:

  • The patient received short-term oral steroid treatment for visual field obstruction.
  • Recovery was achieved without larvae appearing on the skin surface, contrary to literature.

Related Experiment Videos

  • The case highlights the importance of considering hypodermiasis despite initial negative diagnostic indicators.
  • Implications:

    • Hypodermiasis diagnosis should be maintained in bovine husbandry regions with suggestive clinical signs.
    • Systematic cattle treatment programs are essential for the eventual eradication of this parasitic disease.
    • This case underscores the need for vigilance in diagnosing rare parasitic infections in humans.
