Area of Science:

  • Oncology
  • Endocrinology
  • Radiology

Background:

  • Incidental adrenal masses are frequently discovered during abdominal imaging.
  • A diagnostic strategy is essential for managing unsuspected adrenal findings.

Observation:

  • A woman presented with pulmonary embolism and incidentally discovered voluminous bilateral adrenal masses.
  • She showed no signs of hormone hypersecretion but had mild primary adrenal failure.

Findings:

  • CT-guided needle biopsy and staging diagnosed primary bilateral adrenal lymphoma, diffuse large B-cell type.
  • The patient achieved remission after 18 months of CHOP chemotherapy, with dramatic adrenal mass reduction.

Implications:

  • Primary adrenal lymphoma is rare, with only 65 reported cases.
  • Histological diagnosis is crucial due to the potential for excellent response to specific therapies.
