A Korean patient died from methicillin-resistant Staphylococcus aureus (MRSA) sepsis despite vancomycin treatment. The MRSA strain showed reduced vancomycin susceptibility, marking the first vancomycin-intermediate S. aureus case reported in Korea.

Area of Science:

  • Infectious Diseases
  • Microbiology
  • Clinical Medicine

Background:

  • Methicillin-resistant Staphylococcus aureus (MRSA) is a significant public health concern, particularly in Korea, due to its high prevalence.
  • Vancomycin is a critical antibiotic for treating MRSA infections, but rising resistance poses a therapeutic challenge.

Observation:

  • A 45-year-old male patient presented with a long-standing pelvic abscess caused by MRSA.
  • The patient received prolonged treatment with vancomycin and teicoplanin.

Findings:

  • The MRSA blood culture isolate demonstrated reduced susceptibility to vancomycin, with a Minimum Inhibitory Concentration (MIC) of 8 microg/ml.
  • Despite aggressive treatment, the patient succumbed to MRSA sepsis.
  • This case represents the first documented instance of vancomycin-intermediate Staphylococcus aureus (VISA) in Korea.

Related Experiment Videos

Implications:

  • The emergence of vancomycin-intermediate MRSA strains in Korea necessitates urgent surveillance and treatment guideline revisions.
  • This finding highlights the critical need for alternative therapeutic strategies against vancomycin-resistant MRSA.
  • Clinicians should be vigilant for reduced vancomycin susceptibility in MRSA infections, especially in high-prevalence regions.
