    Background:

    • Portal hypertension often necessitates portosystemic anastomosis to prevent gastro-intestinal bleeding.
    • This surgical procedure, however, is associated with significant complications, notably encephalopathy and encephalo-myelopathy.

    Observation:

    • Encephalopathy and encephalo-myelopathy, despite differing clinical and pathological profiles, share a common, not fully understood, pathogenesis.
    • These neurological complications are linked to metabolic disturbances from gut-derived substances entering systemic circulation.

    Findings:

    • Portocaval anastomosis in rats induced Type II Alzheimer's glia in the cerebellum.
    • Spinal cord lesions have not been consistently reproduced in experimental models following this procedure.

    Implications:

    • The common pathogenesis of these neurological complications remains an area requiring further investigation.
    • Portal decompression surgery introduces new pathophysiological questions regarding its long-term neurological effects.
