Summary

This study shows that a combined laparoscopic and vaginal approach for uterine myomas is safe and effective. This method simplifies hemostasis and repair for posterior and fundal fibroids compared to purely laparoscopic techniques.

Area of Science:

  • Minimally Invasive Gynecologic Surgery
  • Reproductive Medicine
  • Surgical Oncology

Background:

  • Uterine myomas are common benign tumors affecting women of reproductive age.
  • Symptomatic myomas often require surgical intervention, with various approaches available.
  • Fundal and posterior wall myomas present unique surgical challenges.

Purpose of the Study:

  • To assess the safety and efficacy of a combined laparoscopic and vaginal approach for uterine myomas.
  • To compare this combined approach to purely laparoscopic myomectomy for specific fibroid locations.

Main Methods:

  • Retrospective case study conducted at a tertiary care teaching hospital.
  • Involved 31 women undergoing laparoscopic-assisted vaginal myomectomy for symptomatic fundal and/or posterior wall uterine myomas.

Related Experiment Videos

  • Data collected on operating time, blood loss, hospital stay, and complications.
  • Main Results:

    • The combined approach demonstrated favorable outcomes with a mean operating time of 79.19 minutes and mean blood loss of 150 ml.
    • Average length of hospital stay was 3.10 days.
    • No serious complications were reported; only two minor complications occurred.

    Conclusions:

    • Laparoscopic inspection followed by vaginal removal of posterior and fundal uterine myomas facilitates easier hemostasis and uterine repair.
    • This combined approach offers a safe and effective alternative for managing challenging uterine fibroids.
    • The technique may improve surgical efficiency and patient recovery.
