Benign paroxysmal positional vertigo of the horizontal semicircular canal (BPPV-HSC) does not affect postural control. However, posterior semicircular canal dysfunction (BPPV-PSC) leads to persistent postural deficits, detectable by dynamic posturography.

Area of Science:

  • Vestibular system disorders
  • Neurology
  • Otolaryngology

Background:

  • Benign paroxysmal positional vertigo (BPPV) is a common vestibular disorder.
  • BPPV affects different semicircular canals, including the horizontal (HSC) and posterior (PSC) canals.
  • The impact of HSC and PSC BPPV on postural control requires further investigation.

Purpose of the Study:

  • To compare postural control in patients with BPPV-HSC and BPPV-PSC.
  • To evaluate the effectiveness of dynamic posturography (DP) in detecting postural deficits.
  • To explore potential causes for residual postural impairment after treatment.

Main Methods:

  • Investigated 16 BPPV-HSC patients, 40 BPPV-PSC patients, and 20 healthy controls.
  • Utilized dynamic posturography (DP) and bithermal caloric stimulation.

Related Experiment Videos

  • Administered Lempert's manoeuvre for treatment and assessed patients pre- and post-intervention.
  • Main Results:

    • No significant postural deficits were found in BPPV-HSC patients compared to controls.
    • BPPV-PSC patients exhibited significantly impaired postural scores compared to BPPV-HSC patients.
    • Residual postural impairment persisted in BPPV-PSC patients even after successful treatment.

    Conclusions:

    • Horizontal semicircular canal disorders do not impact postural control.
    • Dynamic posturography effectively identifies postural imbalance from PSC dysfunction post-vertigo resolution.
    • Residual deficits in BPPV-PSC may stem from canal debris, receptor issues, or central re-adaptation time.
