Adding triiodothyronine (T3) to thyroxine (T4) therapy significantly improved mood in a patient with long-standing depression and hypothyroidism. This suggests T3 may be crucial for treating depression linked to thyroid issues.

Background:

  • Standard thyroxine (T4) replacement therapy may not fully resolve depressive symptoms in all hypothyroid patients.

Observation:

  • A 50-year-old woman with a history of depression and hypothyroidism showed persistent symptoms despite high-dose T4 treatment.
  • Thyroid-stimulating hormone levels remained elevated, indicating inadequate thyroid hormone replacement.

Findings:

  • Addition of a low dose of triiodothyronine (T3) to her T4 regimen led to significant mood improvement.
  • This case suggests T3 may be more effective than T4 alone for treating depression associated with hypothyroidism.
