Background:

  • Castleman's disease is a rare lymphoproliferative disorder.
  • Unicentric Castleman's disease (UCD) typically presents in the chest or abdomen.
  • Pararenal space UCD is exceptionally rare.

Observation:

  • A patient presented with hydronephrosis due to a pararenal mass.
  • The mass had a malignant clinical appearance.
  • The patient underwent surgical resection of the tumor.

Findings:

  • The resected tumor was diagnosed as unicentric Castleman's disease.
  • The patient experienced no recurrence two years post-surgery.
  • This is the third reported case of pararenal UCD associated with hydronephrosis.

Implications:

  • Conservative surgery may be sufficient for pararenal UCD.
  • Radical surgery might be avoidable in select UCD cases.
  • Early diagnosis and limited resection can lead to favorable outcomes.
