Summary

Women experiencing recurrent spontaneous miscarriage (RSM) show a higher prevalence of antithyroid antibodies (ATA), regardless of antipaternal antibodies (APCA) status. This suggests ATA may indicate maternal immune system dysregulation in RSM.

Background:

  • Recurrent spontaneous miscarriage (RSM) is a complex condition with suspected autoimmune and alloimmune factors.
  • Antithyroid antibodies (ATA) and antipaternal antibodies (APCA) are potential indicators of immune system involvement in reproductive failure.

Purpose of the Study:

  • To investigate the frequency of ATA in women with RSM.
  • To assess the role of alloimmune factors, indicated by APCA, in RSM.
  • To determine if ATA prevalence differs in RSM patients with or without APCA.

Main Methods:

  • A case-control study comparing 30 women with RSM (≥3 consecutive miscarriages) and 15 matched controls.
  • Measurement of ATA (thyroid peroxidase and thyroglobulin antibodies) using chemiluminescence immunoassay.
  • Evaluation of APCA using a cross-match reaction.

  • Women with RSM had a significantly higher frequency of ATA (37%) compared to controls (13%, p < 0.05).
  • 67% of women with RSM tested negative for APCA, suggesting alloimmune contribution.
  • Even in the subgroup of RSM patients negative for APCA, ATA frequency remained higher than controls (40% vs 13%, p < 0.05).

Conclusions:

  • A higher frequency of ATA is observed in women with RSM, irrespective of their APCA status.
  • Antithyroid antibodies may serve as a potential biomarker for impaired maternal immune system regulation in recurrent spontaneous miscarriage.
