    Background:

    • Carbenicillin therapy in ARF can be complicated by significant bleeding diathesis.
    • Achieving therapeutic carbenicillin levels in ARF requires careful monitoring and dose management as renal function changes.
    • Pseudomonas and Serratia marcescens were identified as causative agents.

    • Clinicians must consider the risk of bleeding when prescribing carbenicillin to patients with ARF.
    • Optimal dosing strategies for carbenicillin in ARF need further investigation to ensure efficacy and safety.
    • This case series underscores the importance of individualized treatment protocols for severe infections in renal impairment.
