Summary

Symptomatic carpal bone cystic lesions, including juxta-articular bone cysts, were treated with curettage and bone grafting. Favorable outcomes suggest minor trauma initiates these intraosseous lesions.

Area of Science:

  • Orthopedic Surgery
  • Hand Surgery
  • Radiology

Background:

  • Cystic lesions of carpal bones can cause symptoms and require surgical intervention.
  • Accurate diagnosis, understanding etiology, and effective treatment are crucial for managing these conditions.

Observation:

  • Reviewed six cases of symptomatic carpal bone cystic lesions treated surgically.
  • Lesions affected the scaphoid, lunate, capitate, and triquetrum, with multiple lesions in four patients.
  • Diagnoses included juxta-articular bone cyst and bone cyst-like pathologic changes.

Findings:

  • Juxta-articular bone cysts presented as intraosseous cavities with gelatinous material.
  • Bone cyst-like changes showed fibrous connective tissue histologically similar to juxta-articular bone cysts.

Related Experiment Videos

  • All six cases were treated with curettage and cancellous bone grafting, resulting in favorable outcomes without complications.
  • Implications:

    • Minor carpal trauma may initiate intraosseous lesions through intramedullary metaplasia.
    • This process can lead to fibrous connective tissue proliferation and cystic cavity formation.
    • Surgical treatment involving curettage and bone grafting is effective for symptomatic carpal bone cystic lesions.
