Pediatric non-hematopoietic malignancies rarely spread to the skin, often presenting as the initial sign of disease. Rhabdomyosarcoma and neuroblastoma are the most common culprits in children.

Background:

  • The spectrum of cutaneous metastasis from non-hematopoietic neoplasms in pediatric patients is not well-documented.
  • This study examines the histologic diversity of this rare process over three decades at a specialized pediatric cancer center.

Purpose of the Study:

  • To document the histologic types of non-hematopoietic malignancies that metastasize to the skin in children.
  • To identify the primary pediatric tumors most frequently associated with cutaneous metastasis.

Main Methods:

  • A retrospective review of pathology accessions from skin biopsies and autopsies was conducted.
  • Cases coded for metastatic non-hematopoietic malignancy were identified and analyzed.

Main Results:

  • Out of 1,971 skin-related pathology accessions, 40 cases (2%) showed metastatic non-hematopoietic malignancy.
  • The most frequent diagnoses included neuroblastoma (8 cases) and various types of rhabdomyosarcoma (14 cases).
  • Other diagnoses included osteosarcoma, choriocarcinoma, Ewing's sarcoma, and malignant melanoma.

Conclusions:

  • Cutaneous metastasis of non-hematopoietic malignancies in children is uncommon but can be the initial presentation.
  • Rhabdomyosarcoma and neuroblastoma are the primary pediatric tumors most likely to metastasize to the skin.
  • Pediatric tumors are more prone to widespread dissemination to multiple distant sites compared to adult malignancies.
