Background:

  • Congenital thoracic ectopic kidney is an extremely rare developmental anomaly, representing the rarest form of ectopic kidneys.
  • Often asymptomatic, it is typically discovered incidentally during routine chest radiography.

Observation:

  • A case of thoracic ectopic kidney in a 19-month-old boy presented as a well-demarcated mass at the right lung base on chest X-ray.
  • Imaging with intravenous pyelography (IVP) and thoraco-abdominal computed tomography (CT) confirmed a normally functioning transdiaphragmatic thoracic ectopic right kidney.

Findings:

  • Technetium-99m DTPA and DMSA scintigraphy revealed pelvic stasis, indicating functional impairment despite normal anatomical visualization.
  • This highlights the importance of functional imaging in diagnosing ectopic kidney conditions.

Implications:

  • Thoracic ectopic kidney should be included in the differential diagnosis for well-demarcated thoracic masses.
  • Renal scintigraphy is essential for diagnosis, even when CT and IVP scans appear normal, to detect functional abnormalities.
