Women with recurrent pregnancy loss (RPL) have a similar incidence of diminished ovarian reserve (OR) as other infertile patients. Ovarian reserve screening is recommended for RPL work-up before treatment.
Area of Science:
- Reproductive Endocrinology
- Infertility Research
Background:
- Recurrent pregnancy loss (RPL) affects a significant number of women.
- Diminished ovarian reserve (OR) is a known factor impacting fertility outcomes.
- The relationship between RPL and OR requires further clarification.
Purpose of the Study:
- To investigate the incidence of diminished ovarian reserve (OR) in patients experiencing recurrent pregnancy loss (RPL).
- To compare OR status between women with and without RPL.
- To evaluate the impact of OR on reproductive outcomes in RPL patients.
Main Methods:
- Retrospective chart review of 692 women undergoing fertility evaluation at a tertiary fertility center.
- Utilized the clomiphene citrate challenge test (CCCT) to assess ovarian reserve.
- Measured Follicle-Stimulating Hormone (FSH) levels on days 3 and 10 of the menstrual cycle.
Main Results:
- The incidence of abnormal CCCT results was similar in women with RPL (18%) and without RPL (18%).
- Women with RPL were younger but showed similar menstrual cycle lengths compared to the non-RPL group.
- Delivery rates were poor (<5%) for both groups when the CCCT was abnormal, but similar for normal OR patients.
Conclusions:
- Women with RPL exhibit a comparable incidence of diminished ovarian reserve to the general infertile population.
- Poor reproductive outcomes are observed in both RPL and non-RPL patients with abnormal ovarian reserve.
- Ovarian reserve screening is advised in the management of RPL prior to treatments like immunotherapy or anticoagulation.
