Observation:
- A 19-year-old male presented with a 3-year history of dysphagia and regurgitation.
- Imaging revealed a large posterior mediastinal mass encasing the esophagus.
- Endoscopy showed multiple submucosal nodules with intact mucosa.

- Histological examination confirmed diffuse leiomyomatosis of the esophagus.
- The condition presented as multiple confluent myomatous nodules circumferentially involving the esophagus.
Implications:
